Hospice CNA ( Kane, Lake & McHenry Counties)

Solace Hospice & Palliative Care

is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to provide personal care and emotional support to patients in their homes, assisted living communities, and skilled nursing facilities. This role is ideal for CNAs who value meaningful relationships, independence in the field, and a supportive interdisciplinary team.

What You’ll Do

• Provide personal care including bathing, grooming, oral care, toileting, and repositioning.
• Support patient comfort, dignity, and safety in alignment with the individualized plan of care.
• Observe and report changes in condition to the RN Case Manager.
• Offer emotional support to patients and families with professionalism and empathy.
• Assist with light tasks related to patient care (linen changes, tidying care areas).
• Document visits accurately and promptly in the EMR.
• Attend Interdisciplinary Team (IDT) meetings twice per month as required by hospice regulations.
• Collaborate with the clinical team to ensure seamless, compassionate care.

What We’re Looking For

• Active Illinois CNA certification in good standing.
• Valid driver’s license, reliable transportation, and proof of auto insurance.
• Current CPR certification (or willingness to obtain).
• Experience in hospice, home health, or long-term care preferred.
• Strong communication skills, emotional steadiness, and a heart for end‑of‑life care.
• Comfort traveling throughout our service area.
