Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ health by observing vital signs, medication adherence, and overall condition, reporting any concerns promptly
• Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
• Utilize specialized equipment such as Hoyer lifts or transfer devices to ensure safe patient handling and mobility
• Maintain accurate documentation of care plans, patient progress, and incident reports in compliance with HIPAA regulations
• Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment

Requirements

• Proven experience in home care, assisted living, nursing home settings, or group homes with a strong understanding of senior care and disability support
• Knowledge of patient observation techniques, medication administration, vital signs monitoring, and dementia or Alzheimer’s care
• Certifications in CPR and First Aid are required; additional certifications such as HIPAA training or hospice care are a plus
• Ability to lift heavy objects safely and perform physically demanding tasks as needed for patient transfers or mobility assistance
