Heavens Hands Home Care is looking for a dedicated CNA and Home Care Aide to join our team, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes or assisted living environments. Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe clients health status and report any changes in condition
• Provide medication reminders and assist with medication administration following proper protocols and documentation standards
• Support clients with specialized needs such as dementia, Alzheimer’s, or developmental disabilities through tailored care strategies and compassionate engagement
• Operate care equipment such as Hoyer lifts for safe transfers, wheelchairs, gate belt, etc
• Prepare nutritious meals and assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
• Document all patient interactions and medical information in compliance with HIPAA regulations, ensuring confidentiality at all times

Requirements

• Proven experience in home care , senior care, assisted living, nursing homes, or long-term care environments
• Certification in CPR and first aid preferred; additional certifications such as hospice care or dementia training are preferred
• Ability to work independently while following detailed care plans and protocols
• Strong patient observation skills to identify changes in care status promptly
• Experience working with individuals with disabilities or developmental challenges is highly valued
