Job Summary

Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ vital signs and observe changes in health status to report to healthcare professionals
• Prepare nutritious meals tailored to dietary needs and assist with medication administration following care plans
• Operate mobility aids such as Hoyer lifts and ensure safe transfer techniques to prevent injury
• Maintain a clean and safe environment through regular cleaning, laundry, and organization of living spaces
• Document patient observations, care provided, and any incidents accurately in compliance with HIPAA regulations
• Support clients with cognitive impairments such as Alzheimer’s disease or dementia through memory care techniques and engagement activities

Experience

• Prior experience in home health, assisted living, nursing homes, or group homes preferred
• Demonstrated ability to work with individuals with developmental disabilities or autism spectrum disorder
• Knowledge of patient monitoring, vital signs collection, and basic nursing skills such as CPR and first aid certification required
• Experience working with residents who have dementia or require hospice or hospice-like care is a plus
