Duties

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using manual handling tools like Hoyer lifts when necessary
• Monitor patients’ health by taking vital signs, observing for changes, and reporting findings to healthcare professionals
• Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation
• Administer medications accurately following care plans and maintain detailed documentation in compliance with HIPAA regulations
• Support clients with cognitive challenges including dementia, Alzheimer’s, autism, or developmental disabilities through engaging activities and memory care techniques
• Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment
• Document patient progress, update care plans regularly, and collaborate with healthcare teams to ensure comprehensive support

Experience

• Prior experience working in home health, assisted living, nursing homes, or group homes is highly preferred
• Demonstrated ability to work with individuals with disabilities or neurodevelopmental disorders such as autism or intellectual disabilities
• Knowledge of patient observation techniques, medication administration, and vital signs monitoring
• Familiarity with dementia care, Alzheimer’s care, memory care strategies, and hospice support is advantageous
