Job Overview

We are urgently hiring dedicated Spanish-speaking Home Health Aides and Certified Nursing Assistants (CNAs) to provide compassionate in-home and community-based care. This role involves supporting patients with a variety of healthcare needs, including assistance with daily activities, medication management, and health monitoring. Responsibilities

• Provide personalized patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
• Monitor and record vital signs accurately and report changes to healthcare professionals
• Assist with medication administration following prescribed protocols
• Support patients with Alzheimer's, dementia, hospice, or memory care needs using specialized techniques
• Operate Electronic Health Records (EHR/EHR systems) to document patient interactions and medical information securely in compliance with HIPAA regulations
• Assist with medical procedures such as catheter care and Hoyer lift transfers
• Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation
• Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for patients
• Document medical records accurately and ensure compliance with healthcare standards
• Collaborate effectively with healthcare teams to coordinate patient care plans in assisted living, nursing homes, or home settings
