• Assist residents or patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature regularly
• Provide medication reminders and assist with medication administration following care plans and HIPAA regulations
• Support patients with dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement
• Prepare nutritious meals tailored to individual dietary needs, ensuring proper food safety and cleanliness standards
• Maintain detailed documentation of patient observations, care activities, and progress notes in compliance with healthcare protocols
• Utilize equipment such as Hoyer lifts or resident’s <PERSON> to safely transfer individuals requiring heavy lifting or assistance with mobility
• Conduct routine cleaning of living spaces to uphold hygiene standards and promote a healthy environment
• Collaborate with healthcare professionals to develop and implement personalized care plans that meet each individual’s unique needs
• Engage in patient monitoring to observe changes in condition or behavior, reporting concerns promptly to supervisors or medical staff
