Caregiver

Job Summary

We are seeking a compassionate and energetic Caregiver to provide exceptional support and assistance to individuals in various care settings. Direct Care Professional provides an array of services which are necessary to service people and their families with needs related to disabilities, mental health or aging to live in community settings and provide companionship services of fellowship, care and protection. Such services may include household work related to the care such as meal preparation, bed making, washing clothes and other similar services. Supports are individually planned and tailored to meet the specific needs of the people we serve. This may include working at multiple homes and locations.

Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records
• Prepare nutritious meals and assist with meal planning in accordance with dietary restrictions and care plans
• Support medication administration following HIPAA guidelines and care protocols
• Maintain a clean and safe environment by performing light housekeeping, laundry, and sanitation tasks
• Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to ensure safe transfers and mobility assistance
• Document patient observations, update care plans, and communicate effectively with healthcare teams to ensure continuity of care
• Keep accurate daily documentation of activities, Fire/ Disaster drills, behaviors Support Plans, medications, ISP tracking, as well as other documentation and enter into documentation system as required
• Review medication administration reports for assigned homes.
