Whether working in a group home, assisted living facility, or providing home health services, you’ll be at the forefront of delivering personalized care that promotes independence and well-being.

Responsibilities

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and manual handling techniques.
• Monitor patient health by observing vital signs, reporting changes, and maintaining accurate documentation in compliance with HIPAA regulations.
• Prepare nutritious meals tailored to individual dietary needs while ensuring proper food safety and sanitation standards.
• Support medication administration and ensure adherence to care plans under supervision or as trained.
• Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols.
• Observe residents’ behaviors and condition, especially those with dementia, Alzheimer’s, autism, or other neurodevelopmental disabilities, providing appropriate support and engagement.
• Collaborate with healthcare professionals to implement care strategies for residents with complex needs such as hospice or memory care.

Skills

• Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments.
• Strong patient observation skills to monitor health status and respond promptly to emergencies.
