Responsibilities

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and other assistive devices.
• Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, ensuring accurate documentation within Electronic Health Record (EHR) systems .
• Support medication reminders per the resident service plan.
• Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral health management.
• Assist with basic life support during emergency situations.
• Maintain a clean and safe environment by following infection control protocols and proper medical documentation practices.

Skills

• Proven experience working in various healthcare settings such as hospitals, nursing homes, assisted living facilities, supportive living, dementia care or hospice care.
• Knowledge of medical terminology, anatomy, physiology, and aseptic techniques essential for delivering safe patient care.
• Ability to perform patient service tasks including vital signs measurement, medication assistance, and basic life support certification.
