Position Overview

As a CNA or LPN, you will provide personalized care while promoting dignity, safety, and independence. You will work closely with clients, families, and the healthcare team to ensure exceptional care and support.

Responsibilities

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility.
• Safely assist with transfers using gait belts, Hoyer lifts, and other mobility equipment.
• Monitor vital signs and observe for changes in client condition.
• Provide medication reminders (CNAs) or medication administration as permitted by licensure and agency policy (LPNs).
• Provide care for clients with Alzheimer's disease, dementia, memory care needs, hospice care, and other chronic conditions.
• Perform skilled nursing tasks within scope of practice (LPNs), including wound care, catheter care, and other delegated nursing duties.
• Complete accurate and timely daily care documentation using an electronic documentation system.
• Report changes in clients' physical, emotional, or mental condition.
• Assist with light housekeeping, meal preparation, laundry, companionship, and other tasks outlined in the care plan.
• Maintain HIPAA compliance and follow infection control and safety procedures.

QualificationsCNA Applicants

• Current Virginia CNA certification in good standing.
• Current CPR/BLS certification.
• Home care, long-term care, or hospital experience preferred.
