Responsibilities

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support.
• Monitor and record vital signs, medication administration, and changes in patient condition accurately using Wellsky.
• Support patients with medication management while adhering to HIPAA regulations to ensure confidentiality and privacy.
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments, including engaging in therapeutic activities and ensuring safety.
• Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met.
• Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams to ensure continuity of care.

Qualifications

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
• Proven experience working in assisted living facilities, nursing homes, or home health environments.
