Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; document findings accurately using Electronic Health Record (EHR) systems
• Support medication administration following prescribed schedules and medical terminology guidelines
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments; deliver memory care and hospice support as needed
• Perform catheter care and manage other medical procedures with precision and adherence to home healthcare regulations
• Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients
• Utilize assistive devices like Hoyer lifts or transfer belts to ensure safe patient mobility and prevent injury during transfers

Requirements

• Proven experience in assisted living facilities, nursing homes, or home healthcare settings; familiarity with geriatric care and working with individuals with disabilities or developmental disabilities
• Knowledge of medical terminology, medication administration protocols, and vital signs monitoring
• Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS); first aid training is a plus
