Home Health Aide & Caregiver

Overview

We are seeking compassionate and dedicated Home Health Aides to provide exceptional in-home care to clients across various settings. This role involves assisting individuals with daily activities, ensuring their safety, and supporting their overall well-being. Ideal candidates will have a strong background in patient care, experience working with seniors or individuals with disabilities, and a genuine desire to improve quality of life through personalized caregiving. Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ conditions, reporting any changes or concerns promptly
• Administer medications following prescribed schedules and protocols
• Perform medical tasks such as taking vital signs, catheter care, and assisting with medical equipment like Hoyer lifts
• Support patients with Alzheimer's, dementia, memory care, hospice needs, and other specialized care requirements
• Maintain accurate medical documentation using EMR/EHR systems while adhering to HIPAA regulations
• Prepare nutritious meals and assist with meal planning in accordance with dietary restrictions
• Perform light housekeeping duties including cleaning and laundry to maintain a safe and sanitary environment
• Provide emotional support and companionship to promote mental well-being
• Document patient progress and communicate effectively with healthcare teams

Experience

Candidates should possess prior experience in home health or long-term care environments, including assisted living facilities or nursing homes.
