Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts.
• Monitor patient health by observing vital signs and reporting changes to healthcare providers promptly.
• Administer medications and manage medical documentation in compliance with HIPAA regulations and EMR/EHR systems.
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques.
• Support patients with medical procedures such as catheter care, wound care, and hospice-related needs while ensuring comfort and dignity.
• Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical instructions.
• Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and general cleaning.
• Document patient progress accurately in medical records and communicate effectively with interdisciplinary teams to ensure continuity of care.
• Use proper techniques for patient transfers and mobility assistance utilizing equipment like Hoyer lifts or other assistive devices.
