Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Monitor and observe patients’ health status by recording vital signs such as blood pressure, pulse, temperature, and respiration.
• Administer medication and perform catheter care following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient privacy.
• Support clients with medical procedures such as wound care, hospice care, Alzheimer's and dementia care, and memory care activities.
• Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information accurately and efficiently.
• Use assistive devices like Hoyer lifts safely to transfer clients with mobility challenges while ensuring their safety and comfort.
• Prepare nutritious meals aligned with dietary restrictions and assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a healthy living environment.
• Collaborate effectively with healthcare teams by maintaining detailed medical documentation and communicating changes in patient condition promptly.
• Support clients in community settings by providing home & community care that fosters independence and enhances quality of life.
