Responsibilities

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Support patients with specialized needs such as Alzheimer's care, dementia care, hospice support, or working with individuals with developmental disabilities or disabilities
• Prepare nutritious meals and assist with feeding while ensuring safety protocols are followed
• Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for the patient
• Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care and effective communication

Experience

• Prior experience in home health care or long-term care environments is highly preferred
• Proven ability to work with individuals with disabilities, dementia, or memory care needs
• Familiarity with medical terminology, medication administration, catheter care, and hospice care practices
• Demonstrated skills in patient observation, caregiving for seniors or those in assisted living settings, and working within home & community care environments
• Strong understanding of safety procedures related to Hoyer lifts, vital signs monitoring, and infection control standards
