Duties

• Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
• Monitor and observe patient conditions, including vital signs and overall health status
• Administer medications and perform basic medical tasks following healthcare plans and protocols
• Support patients with specialized needs such as Alzheimer's care, dementia care, hospice care, and end-of-life support
• Operate Electronic Health Record (EHR) systems to document medical information accurately and securely
• Use assistive devices like Hoyer lifts and perform catheter care safely and hygienically
• Prepare nutritious meals tailored to individual dietary requirements and assist with light housekeeping tasks like cleaning and laundry

Requirements

• Proven experience in assisted living, nursing homes, or home healthcare environments
• Knowledge of medical terminology, medication administration, and medical documentation standards
• Ability to work with individuals with developmental disabilities or geriatrics with compassion and patience
• Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS) required; First Aid certification preferred
• Familiarity with home & community care regulations, HIPAA privacy standards, and home health agency protocols
