Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes to supervising nurses or physicians
• Provide medication reminders and assist with medication administration following medical instructions and regulations
• Support clients with specialized care needs such as Alzheimer’s disease, dementia, hospice, or end-of-life care
• Operate Electronic Health Record (EHR) systems to document patient information accurately and maintain medical records in compliance with HIPAA regulations
• Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment
• Use medical terminology appropriately when documenting care activities and communicating with healthcare teams
• Safely utilize equipment such as Hoyer lifts or other mobility aids to assist with patient transfers while adhering to safety protocols

Experience

• Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred
