Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patient conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
• Administer medications and treatments following medical instructions while maintaining strict adherence to HIPAA privacy regulations
• Perform vital sign checks such as blood pressure, pulse, temperature, and respiration rates to track health trends
• Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities
• Utilize assistive devices like Hoyer lifts safely to transfer patients and prevent injury
• Manage catheter care, wound care, and hospice support as required by the care plan
• Prepare nutritious meals and assist with meal planning while maintaining food safety standards
• Conduct light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients
• Document all patient interactions accurately in medical records or documentation systems while respecting confidentiality standards
