Responsibilities

• Deliver comprehensive patient observation to monitor health status and report changes promptly.
• Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support.
• Administer medications accurately following medical instructions and document medication records diligently.
• Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates to track patient health.
• Support patients with specialized care needs such as Alzheimer’s, dementia, memory care, hospice support, and catheter management.
• Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions and medical data securely in compliance with HIPAA regulations.
• Use assistive devices like Hoyer lifts safely to prevent injury during transfers and mobility assistance.
• Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and cooking.
• Conduct light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall health and hygiene.
• Collaborate effectively with healthcare teams by updating medical documentation and communicating patient progress clearly.
• Support patients’ emotional well-being through compassionate interaction and engagement in social activities.
