Responsibilities

• Provide attentive patient observation to monitor health status and report changes promptly
• Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Administer medication as prescribed and document medication administration accurately in electronic health record (EHR) systems
• Support patients with medical procedures such as catheter care, vital signs monitoring, and basic life support (BLS) or CPR when necessary
• Offer specialized care for individuals with dementia, Alzheimer’s disease, or other cognitive impairments, including memory care strategies
• Assist with end-of-life and hospice care, ensuring comfort and dignity for patients in their final stages of life
• Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment

Qualifications

• Proven experience working with individuals with developmental disabilities or in assisted living environments
• Knowledge of medical terminology and home & community care regulations
• Prior experience in geriatrics, long-term care, or nursing home settings is highly desirable
• Certification in first aid and CPR required; additional certifications such as Hoyer lift training or hospice care are a plus
