Responsibilities

• Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patient health through observation of vital signs and recognition of changes in condition
• Administer medications accurately following medical instructions and document all medical records in compliance with HIPAA and home healthcare regulations
• Support patients with specialized needs such as dementia care, Alzheimer's care with patience and compassion
• Prepare nutritious meals tailored to dietary requirements and assist with light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment
• Use assistive devices like Hoyer lifts safely to support patient mobility and prevent injury during transfers
• Maintain open communication with healthcare teams using medical terminology to report patient status and updates accurately
• Follow all home & community care standards while providing empathetic support that respects patient dignity and privacy

Qualifications

• Proven experience working as a caregiver in assisted living facilities, nursing homes, or home healthcare settings
• Strong knowledge of medical terminology, medication administration, and vital signs monitoring
• Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease or in hospice/end-of-life care environments
