Memory Care Director LVN

Job Summary

We are seeking a dedicated and compassionate Memory Care Director LVN (Licensed Vocational Nurse) to lead and oversee the specialized care of residents with memory impairments within our assisted living community. In this vital role, you are responsible for providing clinical oversight while leading all Memory Care programming through a person-centered philosophy that promotes dignity, purpose, independence, and quality of life. This position combines nursing leadership with specialized Alzheimer's and dementia programming to ensure residents receive exceptional clinical care while participating in meaningful daily engagement.

The WN/MCD works collaboratively with the Executive Director, Wellness Director (or Company Nurse), physicians, families, hospice, home health, therapy providers, and all departments to create a safe, engaging, therapeutic environment where residents can thrive physically, emotionally, spiritually, cognitively, and socially.

Responsibilities

• Practice within the scope of nursing licensure and state regulations.
• Complete resident assessments and monitor changes in condition.
• Develop, implement, and revise resident service plans.
• Implement physician orders and coordinate follow-up care.
• Maintain infection prevention and quality assurance standards.
• Ensure regulatory compliance and clinical documentation.
• Complete incident investigations and required reports.
• Participate in resident admissions and care conferences.
• Administer medications and treatments safely and accurately.
• Maintain accurate MARs/TARs.
• Monitor medication effectiveness and adverse reactions.
• Process physician orders and pharmacy communications.
• Ensure medication supplies and nursing inventory are maintained.
