Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using manual handling techniques like Hoyer lifts when necessary
• Monitor patients’ vital signs and observe for any changes in health status, documenting findings accurately while adhering to HIPAA regulations
• Prepare nutritious meals tailored to dietary needs and assist with feeding as required, ensuring safety during meal times
• Support clients with cognitive impairments such as dementia or Alzheimer’s disease through memory care strategies and specialized activities
• Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces
• Collaborate with healthcare professionals to develop and implement personalized care plans that address individual needs and goals

Skills

• Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, or in assisted living environments
• Strong patient observation skills to detect changes in condition or behavior promptly
• Knowledge of dementia care, Alzheimer’s care, hospice care, and memory care techniques
• Experience with home health services, long-term care settings, or nursing homes is highly desirable
