Duties

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Monitor patients’ health by observing vital signs, medication administration, and overall condition, ensuring adherence to care plans
• Support residents with meal preparation, feeding, and nutrition needs while maintaining a clean and safe environment
• Document patient observations accurately and update care plans regularly to reflect changing needs
• Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
• Perform light housekeeping duties including cleaning, laundry, and maintaining a hygienic living space
• Follow HIPAA guidelines to ensure patient confidentiality and privacy at all times

Skills

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments
• Strong patient observation skills with the ability to recognize changes in condition or behavior
• Knowledge of dementia care, Alzheimer’s care, memory care, and working with individuals with neurodevelopmental disorders such as autism or intellectual disabilities
• Proficiency in medication administration, vital signs monitoring, CPR/First Aid certification
