Responsibilities

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support
• Monitor and observe patient conditions, including vital signs and overall health status, reporting any changes promptly
• Administer medications and perform basic medical procedures such as catheter care and wound management under supervision or according to care plans
• Support patients with memory care needs, including Alzheimer's and dementia care, ensuring a safe and supportive environment
• Document medical records accurately using Electronic Health Record (EHR) systems while maintaining compliance with HIPAA regulations
• Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
• Utilize equipment like Hoyer lifts or other assistive devices to safely transfer patients and prevent injury
• Provide emotional support and companionship to promote mental well-being during visits

Requirements

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in accordance with state regulations
