Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ health status, recording vital signs such as blood pressure, pulse, and temperature accurately
• Support medication administration following prescribed guidelines while maintaining strict adherence to HIPAA regulations and medical documentation standards
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques
• Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient information securely and efficiently
• Use assistive devices such as Hoyer lifts or catheters to ensure safe patient transfers and hygiene management
• Prepare nutritious meals aligned with dietary needs and assist with light housekeeping tasks like cleaning and laundry to maintain a safe environment
• Collaborate with nursing staff and healthcare professionals to coordinate patient care plans in home or community settings
• Support patients during hospice care by providing comfort measures and emotional support in accordance with care plans
