Job Summary

We are seeking an energetic and dedicated Home Health Aide/Housekeeper to join our team. In this vital role, you will deliver personalized patient care while maintaining a clean, safe, and welcoming environment for our clients. Your compassionate approach and attention to detail will ensure that clients receive the highest level of support, enabling them to live comfortably and safely at home.

Duties

• Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Observe and monitor patients’ conditions, reporting any changes or concerns promptly
• Assist with medication reminders and basic medical tasks such as vital signs monitoring and catheter care
• Maintain a clean, organized living space through cleaning, laundry, cooking, and overall household management
• Support clients with specialized needs such as dementia care, Alzheimer’s care, hospice care, or end-of-life care
• Document patient interactions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems in compliance with HIPAA regulations
• Use appropriate equipment like Hoyer lifts or other assistive devices to ensure safe patient handling

Experience

• Prior experience working with geriatric patients or individuals with developmental disabilities is highly preferred
• Assisted living or long-term care experience is a strong plus
• Familiarity with home healthcare regulations and medical documentation practices is beneficial
