Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools such as Hoyer lifts and other assistive devices.
• Monitor patient health by observing vital signs like blood pressure, pulse, temperature, and respiration; accurately document findings in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
• Support medication administration under supervision, ensuring proper documentation while adhering to HIPAA regulations for patient privacy and confidentiality.
• Provide specialized care for individuals with dementia or Alzheimer’s disease through memory care techniques and tailored activities.
• Assist with medical procedures such as catheter care, wound care, and hospice support as directed by healthcare professionals.
• Prepare nutritious meals and assist with feeding while considering dietary restrictions; perform light cleaning and laundry to maintain a safe and hygienic environment.
• Document all patient interactions thoroughly in medical records, ensuring accuracy for ongoing care coordination.
• Collaborate with nursing staff and other healthcare providers to deliver comprehensive home health services that meet each patient’s unique needs.
