Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and document patient condition through patient observation and medical records using Electronic Health Record (EHR) systems
• Administer medications and perform basic medical tasks such as vital signs measurement, catheter care, and medication administration in accordance with home healthcare regulations
• Support patients with specialized needs such as dementia, Alzheimer's, hospice, or end-of-life care, ensuring comfort and dignity at all times
• Assist with light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment
• Use assistive devices such as Hoyer lifts and work with clients requiring heavy lifting or mobility aids to promote independence and safety
• Maintain HIPAA compliance by safeguarding patient confidentiality during all interactions and documentation

Qualifications

• Proven experience working with individuals with developmental disabilities, geriatrics, or in assisted living environments
• Familiarity with medical terminology, EHR/EHR systems, and medical documentation practices
• Certification in CPR and Basic Life Support (BLS), with additional training in first aid preferred
