Duties

  • Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support
  • Monitor patient health by observing vital signs and reporting changes promptly using Electronic Health Record (EHR) systems
  • Support medication administration following medical instructions and maintaining accurate medical documentation
  • Provide specialized care including Alzheimer's, dementia, memory care, hospice, and end-of-life support
  • Perform catheter care, wound care, and other medical procedures under supervision or as trained
  • Help with light housekeeping tasks such as cleaning, laundry, cooking nutritious meals, and maintaining a safe environment
  • Use assistive devices like Hoyer lifts safely to transfer patients and prevent injury

Requirements

  • Proven experience working as a Home Health Aide or in assisted living/nursing home settings preferred
  • Strong knowledge of medical terminology and home healthcare regulations
  • Ability to perform patient observation accurately and respond effectively to emergencies with First Aid and Basic Life Support (BLS) certification
  • Experience working with individuals with disabilities or developmental delays is highly valued
  • Familiarity with EMR/EHR systems for documentation; HIPAA compliance is essential
