Job Summary

We are seeking a compassionate and dedicated Hospice Home Health Aide to provide exceptional in-home care to patients nearing the end of life. Duties

  • Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to promote independence and comfort
  • Monitor and document patient condition by observing vital signs, behavior, and overall health status, ensuring accurate medical records in electronic health record (EHR) systems
  • Provide medication reminders and assist with medication administration following home healthcare regulations and medical terminology standards
  • Support patients with dementia, Alzheimer’s disease, memory care needs, and those with developmental disabilities or geriatrics by implementing specialized care techniques
  • Perform catheter care, wound care, light housekeeping tasks including cleaning and laundry to maintain a safe and hygienic environment
  • Assist with meal preparation tailored to dietary restrictions and nutritional needs of hospice patients
  • Follow HIPAA guidelines strictly to protect patient privacy and confidentiality at all times
  • Use proper lifting techniques with equipment such as Hoyer lifts for heavy lifting or patient transfers to ensure safety for both patient and caregiver
  • Respond promptly to patient calls or emergencies using basic life support (BLS) skills such as CPR and first aid when necessary
