Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or wheelchairs.
• Monitor patients’ vital signs, observe health status, and report any changes to healthcare professionals promptly.
• Prepare nutritious meals tailored to individual dietary needs, ensuring proper food safety and hygiene standards.
• Administer medications accurately following prescribed care plans while maintaining strict adherence to HIPAA regulations for patient privacy.
• Support clients with memory care needs related to Alzheimer’s disease or dementia by implementing personalized routines and safety protocols.
• Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with healthcare standards.
• Perform light housekeeping tasks such as cleaning, laundry, and organizing to promote a safe and hygienic living environment.

Skills

• Proven experience in home & community care or assisted living environments with a strong understanding of patient observation and monitoring techniques.
• Knowledge of first aid, CPR certification, and emergency response procedures to ensure client safety at all times.
