Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ health status by observing vital signs, medication adherence, and overall well-being
• Prepare nutritious meals in accordance with dietary plans and assist with feeding when necessary
• Support clients with mobility using equipment such as Hoyer lifts and manual handling techniques to ensure safety
• Maintain detailed care plans, document patient observations, and communicate updates to healthcare teams
• Perform light housekeeping duties including cleaning, laundry, and maintaining a safe living environment
• Administer medications accurately following HIPAA guidelines and care protocols while ensuring patient safety

Requirements

• Proven experience in home health, assisted living, nursing homes, or group homes supporting individuals with disabilities or developmental disabilities
• Knowledge of Alzheimer’s care, dementia care, memory care, and hospice care practices
• Ability to work with individuals with autism or neurodevelopmental disorders with patience and understanding
• Strong patient observation skills to monitor health changes and respond appropriately
• Certification in CPR and First Aid is preferred; willingness to obtain relevant certifications if not already held
• Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information
