Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patients’ health status, recording vital signs and reporting any changes to Nurse Supervisor
• Prepare nutritious meals tailored to dietary needs and assist with feeding as necessary
• Support medication administration in accordance with care plans and HIPAA regulations to ensure patient safety
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
• Implement personalized care plans designed by healthcare teams, ensuring each client’s unique needs are met with compassion and professionalism

Requirements

• Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
• Knowledge of patient observation techniques, dementia care (including Alzheimer’s), memory care, and hospice care practices
• Familiarity with HIPAA regulations to ensure confidentiality of patient information
• Ability to perform CPR and First Aid certifications (or willingness to obtain upon hire)
• Strong skills in assisting with ADLs, medication management, patient monitoring, and manual handling techniques
