Responsibilities

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
• Monitor patient health by checking vital signs and observing for any changes in condition
• Administer medications accurately according to care plans while maintaining HIPAA compliance for privacy and confidentiality
• Prepare nutritious meals and assist with meal planning to meet dietary requirements
• Support clients with mobility using equipment like Hoyer lifts or other assistive devices as needed
• Maintain detailed documentation of care provided, patient observations, and any incidents or concerns
• Perform light housekeeping tasks including cleaning, laundry, and organizing to ensure a safe and comfortable environment

Skills

• Proven experience in home & community care or assisted living environments
• Knowledge of patient care techniques including medication administration, patient monitoring, and vital signs measurement
• Familiarity with dementia, Alzheimer's, memory care, hospice care, and working with individuals with developmental disabilities or autism
• Strong understanding of HIPAA regulations and maintaining patient confidentiality
• Ability to perform heavy lifting safely when required for mobility assistance or transfers
• Excellent communication skills to effectively interact with clients, families, and healthcare professionals
