Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, ensuring comfort and dignity at all times
• Monitor patients’ health by observing and report any changes or concerns promptly
• Support medication administration according to care plans while adhering to HIPAA regulations to maintain confidentiality
• Prepare nutritious meals and assist with meal planning to meet individual dietary needs and preferences
• Maintain a clean and safe environment through regular cleaning tasks and proper infection control procedures
• Document care activities accurately in care plans and communicate effectively with team members regarding patient status or changes

Experience

• Previous experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly desirable
• Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, or working with individuals with developmental disabilities enhances your ability to provide specialized support
• Certifications such as HIPAA training or medication administration are a plus
• Familiarity with patient observation techniques and patient monitoring protocols
